Choose your visa category and prepare your documents
Before completing your visa application form, please read the instructions carefully.
Ukrainian citizens can apply for a long-term (D) visa by post if they:
- have a valid work or study invitation,
- own a business in Poland, or
- hold a Polish Card.
Applicants for a local border traffic permit (LBT) must submit their documents in person at the Polish Consulate General.
Make sure you have all required documents for your chosen visa category. You can view the full list here.
From 24 October 2022, all applicants must have travel medical insurance with a minimum coverage of €30,000, valid for the entire stay (for national visas) or for the first entry (for Schengen visas).
Insurance can be purchased at the visa centre or online via IN24, which allows us to verify your policy.
All policies approved by the Polish Ministry of Foreign Affairs are accepted. More information here.
You must also print and complete the Consent to the Processing of Personal Data form and include it with your documents. Form available here.
Important:
- All personal data must match your international passport.
- Sign and date both signature fields on page 3 of your application.
- Applications with incorrect data or missing signatures will not be processed.
