How to submit your Polish visa application

Please read the different steps below to learn how to submit your visa application at the TLScontact visa application centre.​

Complete your application form and book an appointment

Before starting your application, please read these instructions carefully. 

The Visa Application Centre accepts applications for national (type D) and Schengen visas

Applications for other visa categories or local border traffic permits (LBT) must be submitted directly to the Polish Consulate General in Lviv or Lutsk

Important: 

  • Enter your personal details exactly as they appear in your international passport (full name, passport number, date of birth). 
  • Sign and date both fields on page 3 of the application. Include date and place of signature. 
  • Applications with errors or missing signatures will not be processed by the Consulate.

Prepare your documents

Prepare a complete set of documents before your appointment.

From 24 October 2022, travel medical insurance with minimum coverage of €30,000 is required. Insurance must cover your entire stay for national visas or the first entry for Schengen visas.

You can purchase insurance at the Visa Application Centre or online via IN24, which allows us to verify your policy.

All insurance policies approved by the Polish Ministry of Foreign Affairs are accepted. More information here.

You must also:

  • Print, complete and sign two copies of the Consent to the Processing of Personal Data form: Download here.
  • Include the Application for Visa Invalidation, if you have a valid visa that must be cancelled: Download form.

Note:

  • Incomplete applications cannot be processed by the Consulate.
  • Make sure all required documents are included before your appointment.

Submit your application

Bring your documents to the Visa Application Centre on the date of your appointment.Documents are accepted only at the scheduled time, early or late arrivals cannot be accommodated.

When visiting the centre, you can also request optional services such as:

  • Printing and form completion
  • Passport return by courier
  • Travel Insurance
  • Visa Photos service
  • Photocopy

Passport collection

Visa processing takes at least 6 working days (excluding the day of submission).If further review is required, processing may take up to:

  • 30 calendar days for Schengen visas
  • 30 calendar days for national visas

When your passport is ready, you will receive an email notification with collection instructions.

You can:

  • Collect your passport at the Visa Application Centre, or
  • Request courier delivery to your home or to a Meest branch.(Delivery can be requested when submitting your application.)

To collect your passport in person, bring:

  • The TLScontact checklist (received at submission), and
  • The original and copy of your photo ID.

Family members (spouses, parents of minors) may collect passports on your behalf by presenting proof of relationship and the original ID document.

If your passport is not collected within 30 days, it will be returned to the Consulate.
